Beginning with the November 2011 Election, Thomas Township will use a new streamlined AV Application . This “postcard” variety application is designed to make the application process easier for our residents. When you receive your application, it will be folded and tabbed. You should then follow these steps;
1) Keep the application tab shut;
2) Look at your address on the front of the application;
3) Turn the application over and fill in the reason you are requesting an absentee ballot, your date of birth, and sign the application. All of these are highlighted in pink. You can also add your phone number, and email address if you so choose;
4) If you are leaving on vacation there is a place to put an alternate address where you would like your ballot sent, along with a date you are leaving to assist us in determining which address to send your ballot to;
5) Break the tab at the bottom of your application, turn it inside out and tape the application closed again. Place a stamp on the card and mail it back to the Township Clerk;
