Requirements

The Clerk’s Office joined the U.S. Department of State’s “Passport Application Acceptance Program” to assist United States citizens intent on international travel and to protect the integrity of the U.S. passport as proof of U.S. citizenship.

“We are very excited to offer our convenient location for residents and business visitors who need service relating to U.S. passports,” said Thomas Township Clerk, Michael Thayer.
The Western Hemisphere Travel Initiative requires all U.S. citizens traveling by air outside of the United States to present a valid U.S. Passport Book.  During the Summer of 2009, the U.S. Department of State implemented the option of a Passport Card, to expedite entry and document processing at U.S. land and sea ports of entry when arriving from Canada, Mexico, the Caribbean region, and Bermuda.  The Passport Card is not valid for international travel by air.  The Card is designed for the specific needs of border resident communities. 

All individuals who wish to apply for a Passport Book or Passport Card must be present, including infants and children:

    • Completed Passport Application;
    • Original documentation of citizenship (certified birth certificate, previous U.S. passport, naturalization certificate, certificate of citizenship, or report of birth abroad).  The original will get sent in with the application and returned to the mailing address that was completed on the application.
    • Proof of identification (valid driver’s license, U.S. Passport, naturalization/citizenship certificate, or official military identification card);
    • One recent passport photo (2″ x 2″, full-face, on a white or off-white background) see full photo requirements on the application form;
    • Applicable fees.

 

Minors aged 15 and under require both parents’ consent to obtain a passport.  All applicants must appear in person when submitting a passport application.

$100 payable to U.S. Department of State by check or money order

PASSPORT CARD
Adult (16 years and older)
$30 payable to the U.S. Department of State by check or money order
$35 payable by cash, cashier’s check, or money order, payable to Thomas Township

Minor (ages 15 and under)
$15 payable to U.S. Department of State by check or money order
$35 payable by cash, cashier’s check, or money order, payable to Thomas Township

Routine applications are processed within 8-11 weeks; expedited applications are processed within 5-7 weeks from the date received by the Passport Agency for an additional $60.00 (payable by check or money order to the U.S. Department of State). 1-2 Day Delivery is also available for an additional fee, paid per application for 1-2 day delivery of an issued passport book from the Department of State to the customer.  This only applies to mailing addresses within the United States.  Not valid for passport cards. 

Passports for Adults are valid for 10 years; Passports for Minors are valid for 5 years.
